More and more people are wishing to receive home-based support or care in the community, and with an ageing population the demand for these services is expected to rise. We have developed a flexible range of services that provide assistance needed, while still ensuring independence as part of our client-directed care approach. Respect for diversity is paramount in home-based care and our staff are selected to reflect the diversity of clients we support.

The majority of LWB home and community support services was delivered in Queensland in the past 12 months, with over 900 clients.

Professional Care Services

With an ageing population, many older people will be choosing to continue living in their own home, but may need help with housework, meals and personal care, or assistance to do the shopping or attend appointments. Home-based services are not only for older clients, and are accessed by people with a disability, a condition affecting their ability to carry out activities of daily living, or who may be at risk of premature or inappropriate admission to long-term residential care.

We have worked with Commonwealth and State Governments to provide services under the Home Care Packages Program, Home and Community Care Program (HACC), Queensland Community Care Services and Veterans’ Home Care program to deliver personal care, domestic assistance and respite care, and actively support participants to get out and about in their local community.

Our services include:

  • in-home services - assistance with light daily tasks, personal care, home and garden maintenance, home modifications and meals
  • community-based services
  • clinical services - nursing and health services
  • carer support - including respite and counselling
  • day-centre services offering opportunities for social activities.

Home and Community Care programs

We operate these in Queensland, South Australia and the Northern Territory, providing services for people aged 65 years and over, or 50 years and over for Aboriginal and Torres Strait Islander peoples, and people at risk of premature or inappropriate admission to residential care, as well as some carers of older Australians.

Home Care Packages Program

Funded by the Australian Government, our LWB Home Care Packages Program offers eligible people services to maintain independent living in regional New South Wales, Gippsland, Victoria and Alice Springs, Northern Territory.

Veterans’ Home Care program

Funded by the Australian Government’s Department of Veteran Affairs, LWB provides services to eligible veterans, widows or widowers to assist with independent living through the Veterans’ Home Care program in selected regional areas at New South Wales, Queensland, Victoria, Tasmania and Western Australia.

Queensland Community Care Services

Basic support is provided in this state to people under the age of 65 years who would otherwise be unable to live in their own home without care and support.

Day Activity Linking Initiative

In partnership with the Wollongong City Council, LWB provides support to people with a disability aged over 55 years, or over 45 years for Aboriginal and Torres Strait Islander peoples.
